摘要: 目的探讨CT引导下经皮穿刺肺活检术诊断肺占位的技术要领、并发症及预防。方法112例肺占位患者配合“浅呼吸法”行CT引导下经皮穿刺肺活检术,取得病理组织送检,记录穿刺病理结果、并发症情况,并临床随访。结果106例经穿刺病理证实为恶性肿瘤,6例初次病理未查见恶性病理组织,经再次穿刺4例证实恶性肿瘤,2例仍未查见恶性病理组织,经临床随访该2例患者中1例为恶性肿瘤,1例为良性肿瘤,首次病理诊断准确率95.54%(107/112)。24例出现气胸,8例咯血或肿瘤出血,1例出现胸膜反应。结论“浅呼吸法”配合下CT引导肺穿刺活检术具有耐受性好、诊断准确率高、并发症少的优点,值得临床推广,需明确适应证,掌握穿刺技术要领,操作娴熟谨慎。

Abstract: ObjectiveTo explore the technique, complications and preventions of CT guided percutaneous lung biopsy in diagnosing lung neoplasms. Methods112 cases of lung neoplasms were performed CT guided percutaneous lung biopsy with shallow breathing. All pathological results and complications were recorded and then followed clinical followup. Results106 cases were diagnosed malignant tumors in the initial pathology and 6 cases nonmalignant tumor. After second biopsies of the 6 cases, 4 cases were confirmed malignant and 2 cases nonmalignant. Of the 2 “benign” cases, 1 was indicated malignant during followup. The pathological diagnosis accuracy rate was 95.54% (107/112) after the initial biopsies. 24 cases appeared pneumothorax, 8 cases hemoptysis or tumor hemorrhage, 1 cases pleural reaction. ConclusionCT guided lung biopsy is an ideal lung neoplasm diagnostic method with good tolerance, high diagnostic accuracy and  less complications combined with shallow breathing, and it needs clear indications, wellregulated puncture technique,  skilled operation with care.
