分化型甲状腺癌(DTC)是甲状腺癌中最常见的类型,大部分患者具有侵袭性低、预后好的特征。随着医学影像学、细针穿刺活检及分子生物学技术的发展,尤其是外科技术的进步,DTC早期诊断率及手术后生存率明显提高。但是对DTC的认识和外科治疗的某些观点仍存在争议。现结合近40年来的变化对DTC的诊断及外科治疗,特别是DTC原发灶及颈淋巴结转移的治疗策略、手术切除范围和适应证等问题进行系统回顾总结,以期为DTC患者提出更加合理精准的个体化诊疗方案。
